Sick notes & Fitness to work certificates
- If you've been ill less than 7 days: you don't need to see a doctor. You can ask your employer for a self-certification form. If your employer doesn't have one, you can download your own self-certification form on this page. If your employer still asks you for a doctor's note for an illness of less than 7 days, we can do that for you, but there will be a small charge.
- If you've been ill for more than 7 days and you need to continue to stay off work, you'll need a doctor’s certificate, which is called a ‘Fitness to Work’ certificate. You can make an appointment with us to see a doctor, who can help.
- If you need another sick note for the same illness: your doctor may not need to see you to give you another note. Give our reception team a call on 01223 860 387.
